Stephanie, an orientation leader, explains the schedule for the day to a new student at check-in time.
All new students (residents or commuters) need to fill out the appropriate paperwork concerning housing next year. All the forms listed below are available online.
On-Campus Housing Agreement (Resident students only)
Housing Release Form (Commuter students only) This form is required from every student who would like to live off campus. Please remember that there is a freshman/sophomore residency requirement.
Health History Form (All new students need to complete and return this form.)
The required immunizations are:
- tetanus booster within the last 10 years
- proof of two MMRs,
- and a TB test within the year of entering the University
If you fail to meet these requirements, you will not be able to attend classes or check into residence halls.
Recommended immunizations are the Hepatitis B (required for nursing students) and Meningitis vaccines. You are urged to get the required immunizations and completed forms to the Campus Health Center as soon as possible to avoid delaying your start in fall classes.
Insurance Waiver (for students opting out of Capital University Student Health Insurance)
Emergency Contact Form (All new students need to complete this form and return it to Student Affairs.)
Early Arrival Request Form (Resident students only. Please complete and return to Residence Life and Housing.)
If you wish to move in your residence hall early, you will need to complete the early arrival request form. For requested move in time, you may choose the time that works best for you. Please note that if you are approved to move in early, your meal plan will not be begin until Aug. 28. You will receive a letter through the mail once your application is reviewed.
IT informationYour Information Technology information sheet will be included in your folder at orientation. The sheet has information about your e-mail account and the password to access it, as well as your ePay login information so that you can access your e-bill once you receive it through e-mail.
E-BillYou’ll receive a notification via your Capital University e-mail when your electronic billing statement is available. Statements are updated weekly. If you have questions about your e-bill, please consult Student Accounts.
