Special Circumstances

You may have a Special Circumstance which could affect your ability to contribute toward 2009-2010 academic expenses.

The purpose of the Special Circumstance form is to inform us of unusual or extraordinary information that is not included on your FAFSA form. The Department of Education permits college financial aid offices to make adjustments to your FAFSA results for certain circumstances.

To request a Special Circumstance review you will need to:

(1) Print and complete a Verification Worksheet (please include all documents requested on worksheet)

(a) Dependent Verification Worksheet   
                            or 
(b) Independent Verification Worksheet

 

(2) Print the appropriate form (s) below to report your Special Circumstance(s). Please provide all information pertaining to your 2009 income and resources. You can download one or more of the following forms. (All forms require the free Adobe Acrobat reader.)

1. Tuition expenses paid for Private Elementary School, Secondary School, and Parent(s) College

2. Extraordinary Medical and Dental Expenses

3. Disability/Retirement

4. Loss of Income (Job Termination/Job Change/Reduced Hours)

5. Divorce/Separation/Death of Spouse

6. Loss or Reduction of Social Security Benefits, Child Support, or Alimony

7. One-Time-Only Income Distribution Please explain how these funds were used (i.e. Pension, IRA, Capital Gain)

When the requested documentation is received Capital will complete the Federal Verification review and make any necessary adjustments for your Special Circumstance. The student will receive a Financial Aid Award indicating the Special Circumstance review has been completed.
