Forms for New Capital Students
Students entering Capital must return the Health History, Immunization Record and Student Emergency Contact forms BEFORE January 8, 2010.
New Student Health Forms Packet (PDF)This document contains the following health forms: Health History, Meningococcal and Hepatitis B Vaccination Status Form and the Student Immunization Record. Students should download this packet only if all three forms are needed. Each form is available separately if only one is needed.
Student Emergency Contact Information Form (PDF)New Capital students should use this form to notify authorized Capital University health services and Residence Life personnel of personal emergency contact and health information. This form should be completed and returned to Student Affairs by January 4, 2010.
Housing Forms
All students will be required to complete an Online Housing Application or Online Housing Release form.
All Capital students must complete one of the forms below.
(Submit forms no later than January 4, 2010. The priority deadline for the Online Housing Application is December 1, 2009.)
All first and second-year students are required to live in university housing. However, Capital University does recognize that some students may live within the area or have other circumstances that make them eligible to reside off-campus. Please complete and submit the Online Housing Release Form if you are considering living off campus.
Online Housing Application
For new students who plan to live on campus, after your online application is received, you will receive a confirmation letter with a copy of the Campus Housing Application to be signed and returned to the Office of Residence Life and Housing. Students WILL NOT be assigned to campus housing until the Campus Housing Application is received. Returning students will complete the Campus Housing Application during the lottery process.
