If paid or postmarked by May 1, 2014, the Creative Arts Camps fee is $715.
If paid after May 1, the summer camp fee is $765. The fee covers instruction, housing, food, and all program materials. The fee may be paid by check, money order, American Express, Discover, MasterCard, or VISA.
Register by mailing the printable registration form or by submitting the
online registration form.
All registrations must be received by 5 pm EST on Thursday, July 10, 2014.

We will send you a confirmation of your camp registration by email or,
if preferred, by postal mail.
We can issue you a partial refund of your registration fee if we receive
notification no later than 5 pm EST on Thursday, July 17, 2014. Send your written request by fax to
614-236-6169 or by email to email@example.com. A $35 administrative fee will be charged per
cancelled registration fee.
For legal reasons, the University cannot release the names or phone
numbers of other camp participants, not even to those who want to carpool.

Camp staff members will refer medical emergencies and illnesses to
Nationwide Children’s Hospital. A parent or guardian must sign the “Medical
Treatment Authorization” portion of the registration form to grant permission
for any medical attention required during the program.
Fees and Insurance
Capital University does not provide medical insurance for campers.
