New Student-Athlete Medical Forms

We are pleased to welcome you to the Capital University Athletics family. There are a few important items that must be completed and returned so that we can establish your personal, confidential file with the Athletic Training Department. Do not to confuse this paperwork with that of the Campus Health Service. We are a separate entity.

All forms must be read and completed before you will be allowed to participate in any Capital University athletic activity. Please take time to read and complete these forms and return them no later than August 1, 2009. You will not be allowed to participate in any athletic activity until all forms are signed, dated and completed! These items are crucial in medically clearing you for participation in intercollegiate athletics at Capital University establishing your eligibility for coverage under our secondary insurance policy. Incomplete forms will be returned to you, which will delay the process. Remember, this process must be complete before you will be allowed to participate.

1. Athletic Participation Physical (Freshman/Transfer Student)
2. Medical History
3. Disclosure of Protected Health Information
4. Permission to Treat and Consult
5. Assumption of Risk
6. Insurance Form (with copy of card attached)
